Nominate an Outstanding Educator

Purpose of Award: To recognize an educator who has made an impact on education in the Madison community.

Award Criteria:

  • Currently employed or recently retired as an educator in the Madison community
  • Significant contributions to strengthen and/or improve education in the Madison community
  • Personal characteristics that make him/her worthy of the recognition
  • Community involvement as well as involvement in education

Nomination Materials:

  • One letter/email of nomination and one additional letter/email of support. The letter of nomination should specify the award category for the nominee and should address the award criteria listed for that award. The nomination letter should also include contact information on the individual making the nomination (home phone number and address, business phone number and address, email address).
  • Individual: Personal biographical information (or vita) for the nominee, including:
    • Educational background
    • Professional work history
    • Summary of other awards received in recent history
    • Summary of professional affiliations
    • Contact information for nominee (home phone number and address, school phone number and address, email address)
  • Organization: Information on the organization, including:
    • Organizational charter or mission statement
    • Description of membership and organizational history
    • Summary of projects/activities related to education
    • Summary of other service projects (not directly linked to education)
    • Summary of other service awards received in recent history
    • Contact information for organization’s leadership (home phone number and address, work phone number, email address)
  • Photograph of nominee or organization’s membership, suitable for publicity (5 x 7 color, headshot preferred)
